USS Connecticut Final Investigation Report
A long watch, but worth it. Not only for the investigation results, but US Sub Force Operations, Deployment requirements and Command Structure.
Though I can't speak of the CO, XO, NAV and ANAV, as a former QM, I can only say I'm flabbergasted as to why the QM in question would have done what is described here. We may all make mistakes, but cavalier disregard to established SOP?? Also leaves al lot to be said about COMSUBPAC.
